ABSTRACT

RESUMO Introdução: Exostose é um crescimento ósseo benigno que comumente acomete a mandíbula e a maxila, porém raramente apresenta grandes dimensões. Objetivo: Descrever um caso de exostose palatina de apresentação rara e discutir os principais aspectos clínicos envolvidos. Relato de caso: Paciente, gênero masculino, 66 anos, melanoderma, com história de aumento de volume no palato duro com tempo de evolução indeterminado, sem queixas álgicas, porém relatava transtornos de mastigação e fonação. O conjunto da lesão obliterava toda a profundidade do palato e ocupava grande volume na cavidade oral. A Tomografia Computadorizada de Feixe Cônico demonstrava áreas bem corticalizadas na superfície da lesão e áreas centrais menos hiperdensas, compatíveis com osso medular maduro. Após exames e pareceres pós-operatórios o paciente foi submetido a tratamento cirúrgico sob anestesia geral para remoção da massa óssea e osteoplastia subjacente, a fim de dar contornos normais ao palato, e o material colhido foi enviado para a biópsia que estabeleceu o diagnóstico de exostose. O paciente evoluiu sem complicações. Conclusão: Embora a exostose seja um crescimento ósseo benigno, ela causou problemas funcionais ao paciente, necessitando assim de um tratamento cirúrgico. Após três meses da remoção, os distúrbios foram revertidos à normalidade, e a sua reabilitação protética está em andamento(AU)


RESUMEN Introducción: La exostosis es un crecimiento óseo benigno que generalmente afecta la mandíbula y el maxilar, pero rara vez es grande. Objetivo: Describir un caso de exostosis palatina rara y discutir los principales aspectos clínicos involucrados. Presentación del caso: Paciente masculino de 66 años, melanoderma, con antecedentes de aumento de volumen del paladar duro con una evolución de tiempo indefinido, trastornos de masticación y fonación El conjunto de lesiones borró toda la profundidad del paladar y ocupó un gran volumen en la cavidad bucal. La tomografía computarizada de haz cónico mostró áreas corticalizadas en la superficie de la lesión y áreas centrales menos hiperdensas, compatibles con la médula ósea madura. Después de los exámenes y consejos preoperatorios, el paciente se sometió a un tratamiento quirúrgico con anestesia general para eliminar la masa ósea con osteoplastia subyacente a fin de obtener contornos del paladar normales, y el material recolectado fue enviado para realizar biopsia que estableció el diagnóstico de exostosis. El paciente progresó sin complicaciones. Conclusiones: Aunque la exostosis es un crecimiento óseo benigno, causó problemas funcionales al paciente, por lo que requirió tratamiento quirúrgico. Tres meses después de la extracción, los trastornos han vuelto a la normalidad y su rehabilitación protésica está en proceso(AU)


ABSTRACT Introduction: Exostosis is a benign bone growth that commonly affects the jaw and maxilla, but is rarely large. Objective: Describe a case of rare palatal exostosis and discuss the main clinical aspects involved. Case presentation: A case is presented of a male 66-year-old dark-skin patient with a history of hard palate enlargement of an indefinite time of evolution, and chewing and phonation disorders. The lesions had obliterated the entire palatal depth and occupied a large section of the oral cavity. Cone beam computed tomography revealed corticalized areas on the surface of the lesion and less hyperdense central areas compatible with mature bone marrow. After preoperative examination and advice, the patient underwent surgical treatment under general anesthesia to remove the bone mass and underlying osteoplasty to obtain normal palatal contours. The material collected was sent for biopsy testing, which established the diagnosis of exostosis. The patient recovered without any complications. Conclusions: Although exostosis is a benign bone growth, in this case it caused functional problems to the patient, thus requiring surgical treatment. Three months after removal, the disorders have been reverted to normal and prosthetic rehabilitation is underway(AU)

ABSTRACT

O adenoma pleomórfico é a neoplasia, que apresenta maior procedência das glândulas salivares maiores e menores. Clinicamente apresenta-se como lesão solitária, indolor, bem delimitada e de crescimento lento, não se fixando ao tecido adjacente. O tratamento de eleição consiste na excisão cirúrgica da lesão, com pequena margem de segurança. Este trabalho tem como objetivo relatar um caso de adenoma pleomórfico, localizado no palato com quatro anos de evolução de uma pacíente do gênero feminino, faioderma, na quarta década de vida. Inicialmente realizou-se biópsia incisional para ratificação histopatológica da hipótese diagnóstica inicial. No segundo tempo cirúrgico, realizou-se enucleação da lesão e instalação da placa palatina no pós-operatório imediato, melhorando substancialmente o conforto do paciente e a cicatrização da ferida cirúrgica. The pleomorphic adenoma is the neoplasm with the highest incidence of major and minor salivary glands. Clinically presents as a solitary lesion, painless, well outlined and slow growth, not fixing to adjacent tissue. The treatment of choice is surgical excision of the lesion with a small margin of safety. This study aims to report a case of pleomorphic adenoma found on the palate with four years of the evolution of a female patient, brunette skin, in the fourth decade of life. Initially held incisional biopsy for histopathological confirmation of the diagnosis. In the second procedure was performed enucleation of the lesion and installation of the palatal plate in the immediate postoperative period, substantially improving patient comfort and healing of the surgical wound. After a year of follow-up, there is no clinical evidence of recurrence... (AU)

AbstractSurgically assisted rapid maxillary expansion (SARME) is the treatment of choice to adult patients even with severe transversal maxillary discrepancies. However, the adequate retention period to achieve the bone remodeling, thus assuring treatment stability, is controversial.Objective To evaluate the opening pattern and bone neoformation process at the midpalatal suture in patients submitted to surgically assisted (SARME) through cone beam computed tomography (CBCT).Material and Methods Fourteen patients were submitted to SARME through subtotal Le Fort I osteotomy. Both the opening pattern and the mean bone density at midpalatal suture area to evaluate bone formation were assessed pre- and post-operatively (15, 60 and 180 days) through CBCT.Results Type I opening pattern (from anterior to posterior nasal spine) occurred in 12 subjects while type II opening pattern (from anterior nasal spine to transverse palatine suture) occurred in 2 individuals. The 180-day postoperative mean (PO 180) of bone density value was 49.9% of the preoperative mean (Pre) value.Conclusions The opening pattern of midpalatal suture is more related to patients’ age (23.9 years in type I and 33.5 years in type II) and surgical technique. It was not possible to observe complete bone formation at midpalatal suture area at the ending of the retention period studied (180 days).

ABSTRACT

Se presenta el caso de un paciente de sexo femenino de 10 meses de edad referida a la Clínica Dental de la Universidad Cayetano Heredia por presentar una tumoración en el paladar, con un tiempo de enfermedad de 2 meses. Fue examinada por un odontólogo particular el cual le prescribió antibióticos y la derivo a la clínica dental Cayetano Heredia. La evaluó el departamento de cirugía bucomaxilofacial y ellos la derivaron a Odontopediatria. AI examen clínico intraoral se observe una tumoración amarillenta en forma de "frejol" en la línea media del paladar dura rodeado de un rodete de mucosa de coloración normal. El diagnostico presuntivo fue lipoma. Se hizo una interconsulta con Medicina y Patología Oral, llegándose al diagnóstico definitivo de: "objeto extraño", procediéndose procedió al retiro seguido de la limpieza de la zona. This paper reports the case of an 8 months baby girl who was derived to UPCH Dental Clinic. She presented a tumor on the palate for 2 months. First, she was evaluated by a general dentist who prescribed her antibiotics and derived her to UP CH Dental Clinic. She was evaluated by the department of Bucomaxillofacial Surgery who derived the patient to Pediatric Dentistry. By the clinical examination it was noticed a yellow tumor which has bean form in the middle of the hard palate rounded by normal oral mucosa. The initial diagnosis was lipoma. An interconsultation was done with Medicine and Oral Pathology. The definitive diagnosis was foreign body which was taken out. The zone behind it was cleaned and her parents were instructed about how to do it...

INTRODUÇÃO: Tradicionalmente, a fissura palatina é corrigida em duas camadas - uma camada mucosa nasal e camada muco-periosteal oral. Este estudo avaliou os resultados do fechamento em camada única de fissura palatina comparado ao fechamento tradicional em camada dupla. MÉTODOS: Trata se de revisão de prontuários de 101 casos de correção de fissura palatina realizados entre 1981 e 2012 em uma clínica assistencial/hospital terciário localizado no centro de Wisconsin. Os casos utilizaram fechamento em camada única e foram acompanhados em Clínica de Lábio Leporino por 12 meses. Foram incluídas fissura labial e palatina também como fissura palatina isolada. RESULTADOS: Todos os casos apresentaram cicatrização satisfatória exceto dois casos que necessitaram de correção posterior de pequena fistula. CONCLUSÃO: O fechamento em camada única de fissura palatina é tão efetivo quanto o fechamento tradicional em camada dupla, além disso apresenta mínimas complicações.


INTRODUCTION: Traditionally, cleft of the hard palate is repaired in two layers, with a nasal mucosal layer and an oral mucoperiosteal layer. The aim of this study was to evaluate the results of one layer closure of hard palate cleft compared to the traditional two layers closure. METHODS: The charts of 101 consecutive cases of repair of hard palate cleft performed by the authors from 1981 to 2012 at a tertiary care clinic/hospital in central Wisconsin were reviewed. The cases utilized the single layer closure and were followed in the Cleft Palate Clinic on a yearly basis. Cases included unilateral and bilateral cleft lip and palate as well as isolated cleft palate. RESULTS: All cases healed satisfactorily except for two cases that later required small fistulae repair. CONCLUSION: Single layer closure of the hard palate cleft is as effective as traditional two-layer closure, with minimal complications.

ABSTRACT

INTRODUÇÃO: A fístula palatina é a complicação mais frequente após palatoplastias e sua presença traz diversas implicações. Sua apresentação é diversificada e seu reparo pode ser difícil, o que se traduz na existência de uma diversidade de técnicas cirúrgicas descritas. OBJETIVO: Relatar a correção cirúrgica de fístula palatina anterior com retalho miomucoso labial superior, além de fazer uma breve revisão da literatura. RELATO DE CASO: Paciente submetido à correção de fissura palatina completa, apresentando, no pós-operatório mediato, fístula anterior de palato duro e processo alveolar, submetido à correção da fístula palatina oronasal com retalho miomucoso de lábio superior. O paciente evoluiu satisfatoriamente, sem complicações e sem recidiva da lesão após um ano de seguimento. CONCLUSÃO: A técnica em questão mostrou-se simples e eficiente, prestando-se à correção da fístula palatal anterior.


INTRODUCTION: Palatal fistula is the most common complication after palatoplasty, and its presence entails various implications. Its presentation is diverse and repair can be difficult, which is reflected in the existence of a numerous surgical techniques described for its treatment. OBJECTIVE: To report the surgical correction of palatal fistula with a myomucosal upper lip flap, along with a brief review of the literature. CASE REPORT: The patient underwent repair of complete cleft palate. At the immediate postoperative time, an anterior fistula of the hard palate and alveolar process was present and was subjected to a correction with an oronasal myomucosal upper lip flap. The patient progressed satisfactorily, without complications or lesion recurrence after 1 year of follow-up. CONCLUSION: The technique presented is a simple and efficient method for correction of an anterior palatal fistula.

ABSTRACT

El conocimiento estructural y dimensional de las suturas palatinas es necesario para el tratamiento temprano de deficiencias transversales y longitudinales por técnicas de expansión o distracción osteogénica (DO). El objetivo fue estudiar el estado de las suturas palatinas y los cambios cambios dimensionales y morfológicos desde el nacimiento a la niñez. Fueron analizados 41 paladares óseos de ambos sexos entre 0 y 13 años, agrupándolos en recién nacidos (RN/n=17), infantes (IN/n=12) y niños (NI/n=12). Se fotografió cada paladar y se marcaron puntos craneométricos para determinar las dimensiones longitudinales y transversales de las suturas palatinas mediana premaxilar (SPPMX), mediana anterior (SPMA), posterior (SPMP), transversa anterior (SPTA) y transversa posterior (SPTP). Además se evaluó el estado sutural y dimorfismo sexual. Los resultados se sometieron a las pruebas OneWay-ANOVA, Bonferroni y t-test. Las suturas SPMA, SPMP y SPTP no presentaron sinostosis. La SPPMX se observo parcialmente sinostosada en 5,9 por ciento de RN y 16,7 por ciento de NI, y la SPTA en 8,3 por ciento de IN y 41,7 por ciento de NI. Al comparar la longitud sutural en los tres grupos, las dimensiones longitudinales estadísticamente significativas en todos los grupos (p<0,001). No hubieron diferencias en las suturas transversales entre IN y NI (p=0,32). Se observó dimorfismo en IN entre las SPMA y SPTP. El sistema sutural palatino juega un rol importante durante el crecimiento por su disposición sagital y transversal, permitiendo un crecimiento bidireccional del paladar. El crecimiento longitudinal es constante, mientras que el transversal muestra un peak hasta los 2 primeros años de vida y luego disminuye, sugiriendo la existencia de diferentes potenciales de crecimiento. Estas observaciones podrían explicar la alta prevalencia de alteraciones de crecimiento transversal en los niños...


Structural and dimensional knowledge of palatal sutures are necessary for early treatment of deficiencies by transverse and longitudinal expansion techniques or distraction osteogenesis (DO). The aim was to study the status and dimensional or morphological changes of palatal sutures from birth to childhood. Forty one bony palates of both sexes, between 0 and 13 yearsandgrouped in newborns (NB/n = 17), infants (IN/n = 12) and children (CH/n=12) were analyzed. All palates were photographed and craniometrics points were scored to determine the longitudinal and transverse dimensions of the palatal sutures: premaxillary (PMX), anterior midpalatal suture (AMPS), posterior midpalatal suture (PMPS), anterior transverse palatal suture (ATPS) and posterior transverse palatal suture (PTPS). In addition, we evaluated the sutural and sexual dimorphism. The results were subjected to tests OneWay - ANOVAandBonferroni t-test. AMPS, PMPS and PTPS no showed synostosis. PMX was observed partially sinostosed in 5.9% of NB and 16.7% of CH, and ATPS in 8.3% of IN and 41.7% of CH. When comparing the sutural length between groups, the differences were significant in all cases (p <0.001). There were no differences in transverse sutures between IN and CH groups (p=0.32). Sexual dimorphism wasobserved between AMPS and PTPS groups. The palatal suture system plays an important role during growth by sagittal and transverse available, allowing bidirectional growth of the palate. Longitudinal growth is constant, while the cross shows a peak until the first 2 years of life and then decreases, suggesting the existence of different potentials growth. These observations may explain the high prevalence of abnormal transverse growth in children...

Introducción. El torus , o rodete, palatino es una alteración ósea benigna que puede generar molestias en la fonación y en la deglución. Cuando su crecimiento o exposición persistente producen síntomas molestos, debe ser resecado quirúrgicamente. Presentación del caso. Se trata de una paciente de 82 años de edad, que consultó por una lesión ulcerativa de un año de evolución, que producía dolor, halitosis y molestia a la deglución. En el examen físico de la cavidad oral, se observó una prominencia de consistencia dura en la línea media, con perforación y ulceración de la mucosa palatina en su porción lateral posterior izquierda. Se consideraron los diagnósticos de torus palatino, osteoma, granuloma piógeno y neoplasia de tejidos blandos. Se decidió resecarla quirúrgicamente y hacer el estudio histopatológico. La evolución clínica fue satisfactoria, con resolución completa un mes después de la cirugía. En el estudio de histopatología se observó hiperostosis ósea con inflamación crónica de los tejidos blandos e hiperplasia simple de la mucosa, que corresponden a un torus palatino. Discusión. Se reporta un inusual caso de exposición espontánea de un torus palatino que ocupaba casi toda la bóveda palatina, se describe su resección quirúrgica y se informan los hallazgos de histopatología. En la revisión bibliográfica realizada, no se encontró un reporte previo de exposición espontánea de torus palatino.


Introduction: Palatal torus, or torus palatinus, is a benign bone alteration that may cause some discomfort during phonation or swallowing. When its growth or persistent exposition produces unpleasant symptoms, it must be surgically removed. Case presentation: We treated an 82-year-old female patient who consulted for a painful ulcerous lesion she had had for a year and which produced halitosis and discomfort when swallowing. During the oral cavity physical exam we observed a hard protuberance on the midline. It showed a perforation and ulceration of the lateral posterior palatal mucosa on the left side. We considered the following diagnosis: palatal torus, osteoma, pyogenic granuloma, or a soft-tissue neoplasia. We decided to surgically remove it and to perform a histopathological examination. Clinical evolution was satisfactory with complete resolution one month after surgery. The histopathological examination showed hyperostosis along with chronic inflammation of the soft tissues and a simple hyperplasia of the mucosa, which in turn indicated a palatal torus. Discussion: We report an unusual case of spontaneous exposition of a palatal torus which took up almost all of the hard palate area. Its resection is described and we inform the histopathological findings. In the literature review, we did not find a previous report of a spontaneous exposition of a palatal torus.

Maxillary defects are created following surgical treatment of patients with congenital defects, trauma, or neoplasm. Oral cancer is one of the more common malignancies if detected lately massive tissues will be excised surgically and correction will be challenging. One of the treatment strategies is obturator, the retention of which is a major problem. 70 years old female presented with complaint of difficulty in chewing, speech and unlike her appearance. Hemimaxillectomy [or transectomy] was done 6 months ago as surgical treatment of squamous cell carcinoma resulted in a huge hole on the right side of the remaining maxilla.The size of the defect was about 3.0X1.5 cm. Acrylic obturator was made. Soft liner [Mucopren, Germany] was added to the fitting surface of the obturator. The patient's speech, swallowing, mastication and facial appearance were improved. The use of hard acrylic base obturator with soft linear rendered the obturator more retentive and comfortable to the patient. To the best of our knowledge this is the first case to be reported in Sudan in a method different from those used by others worldwide

ABSTRACT

Fibrous dysplasia is a rare disorder of the bone. It is seen in two main forms ofpresentation: monostotic and the polyostotic. A case of monostotic fibrous dysplasia of the maxillary and palatine bones in a 22-year old man who received prosthetic reconstruction is presented with a review of the literature.


La displasia fibrosa es un trastorno raro del hueso. Se le ve en dos formas principales: la monostótica y la poliostótica. Junto con la correspondiente revisión de la literatura, se presenta un caso de displasia fibrosa monostótica de los huesos maxilar y palatino en un hombre de 22 anos que recibió una reconstrucción prostética.

ABSTRACT

La corrección de la deficiencia maxilar transversal (DMT) es indispensable, para lograr una oclusión funcionalmente estable. La disyunción palatina rápida asistida quirúrgicamente o SARPE por su sigla en inglés, es una técnica quirúrgica que ayuda al ortodoncista a resolver este problema en pacientes adultos.A pesar que su utilización esta ampliamente divulgada, desde su introducción por Brown en 1938, existen controversias con respecto a sus indicaciones, sus ventajas con respecto a otras técnicas para ensanchar la maxila, la técnica quirúrgica a utilizar,la causa y la cantidad de su recidiva, su efecto sobre la cavidad nasal, etc.En este trabajo, se realizó una revisión la literatura con la intención de discutir y clarificar algunos puntos donde aun existen controversias.

ABSTRACT

AIMS AND OBJECTIVES: The aim of this study is to determine the effectiveness of subepithelial connective tissue grafts (SCTG) in the coverage of denuded roots. MATERIALS AND METHODS: A total of 16 sites with > or =2 mm of recession height were included in the study for treatment with SCTG. The clinical parameters, such as recession height, recession width, width of keratinized gingiva, probing pocket depth, and clinical attachment level were measured at the baseline, third month, and at the end of the study [sixth month]. The defects were treated with a coronally positioned pedicle graft combined with connective tissue graft. RESULTS: Out of 16 sites treated with SCTG, 11 sites showed complete (100%) root coverage; the mean root coverage obtained was 87.5%. There was a statistically significant reduction in recession height, recession width, and probing pocket depth. There was also a statistically significant increase in the width of keratinized gingiva and also a gain in clinical attachment level. The postoperative results were both clinically and statistically significant ( P 0.05). CONCLUSION: From this study, it may be concluded that SCTG is a safe and effective method for the coverage of denuded roots.

ABSTRACT

O propósito deste estudo foi analisar o comportamento do osso alveolar na remodelação óssea sob carga fisiológica e ortopédica após a disjunção rápida da sutura palatina mediana durante 14 dias, com e sem aplicação de laser de baixa potência e comparar imagens obtidas por técnica radiográfica convencional com microtomografias computadorizadas. O aparelho de laser utilizado foi de diodo de arsenieto de gálio-alumínio (AlGaAs) (830m; 100mW, onda contínua, 126J/cm2, 29 segundos) aplicado à sutura palatina mediana de ratos Wistar, machos, de 6 semanas de vida. Foram utilizados 65 ratos divididos em um grupo controle (GC) contendo 5 ratos e dois experimentais I e II (GE I e GE II), com 30 ratos cada, ambos GE submetidos à disjunção imediata, sendo que o GE II foi irradiado imediatamente após a disjunção. Os GE I e II foram subdivididos em períodos de disjunção imediata, 6, 24, 48 horas, 7 e 14 dias. Dois amostras de cada período dos GC e GE foram radiografadas (convencional) e processadas para captura de imagens em scanner micro-CT (Scanco Medical, Zurik, Suíça), com 24KV de energia e 3,8μm de resolução espacial e o restante foram processadas para serem analisadas histologicamente coradas com hematoxilina e eosina e tricrômico de Masson. Os resultados foram avaliados histológica e radiograficamente. O trabeculado ósseo recém-formado a partir dos bordos ósseos da sutura foi observado nos períodos de 2, 7 e 14 dias do GE II, 7 e 14 dias do GE I. O presente experimento demonstrou que a atividade osteoblástica aumenta a atividade da osteosíntese de modo mais expressivo após aplicação do laser de baixa potência, porem é necessários mais estudos em biologia molecular para desvendar os seus efeitos ao nível celular. As microtomografias computadorizadas mostraram maior riqueza de detalhes para analisar o rompimento da sutura palatina mediana e seu reparo com o depósito de tecido ósseo do que as radiografias convencionais.


The purpose of this study was to evaluate the behavior of the alveolar bone on bone remodeling under physiological load and on after rapid midpalatal suture expansion during a 14-day period, with and without low-level laser irradiation and to compare images obtained by conventional radiographic technique with microtomografias computed. The laser device was a gallium-aluminum-arsenide (GaAlAs) diode laser (830 m; 100 mW, continuous wave, 126 J/cm2, 29 seconds) applied to the midpalatal suture of 6-week-old male Wistar rats. Sixty-five rats were assigned in a control group (CG) containing 5 rats and two experimental I and II (EG I and EG II), with 30 rats each, both EG submitted to expansion immediately, while the EG II was irradiated pos expansion immediately. The EG I and II were divided into periods of immediate midpalatal suture expansion, 6 hours, 1, 2, 7 and 14 days). . Two samples of each period of CG and EG were radiographed (conventional) and processed to capture images in micro-CT scanner (Scancem Medical, Zurik, Switzerland) with 24kV of energy and 3.8 mm in spatial resolution and the rest were processed for are analyzed histologically stained with hematoxylin and eosin and Masson's trichrome.The results were evaluated histologically and radiographically. Bone trabeculae recently formed from the bone borders of the suture were observed in the periods of 2, 7 and 14 days of EG II, and 7 and 14 days of EG I. The present experiment demonstrated that the osteoblastic activity increases the osteosynthesis activity in a more manner after low-level laser irradiation. However, more studies in the field of molecular biology are necessary to determine its effects at cellular level. The computed microtomographs provided a more detailed analysis of midpalatal suture opening and its repair by bone tissue deposition than conventional radiographs.

ABSTRACT

An analytical prospective study was performed to determine the post-operative pain-relieving effect of local steroid suspension injection in uvulopalatopharyngoplasty. From February 2000 to October 2000, 48 adult patients from 20 to 67 years of age, were scheduled to receive uvulopalatopharyngoplasty. Triamcinolone acetonide (Kenacort A) was injected onto the raw surface of the left-sided tonsillar fossa and left-sided soft palate after tonsillectomy and uvulopalatopharyngoplasty. Other preoperative and post-operative medications including antibiotics, anesthesia and surgical techniques were standardized. Visual analog scales were used to assess the level of pain sensation on the left and right side of the throat daily from day 0 (Operative day) to post-operative day 10. A paired t-test showed significant differences in post-operative pain level between the study side (left) and the control side (right) at day 2, day 4, and in the overall analysis (p < 0.05).
